小児医療の業界構造分析と
埼玉県越谷市における戦略展開に関する研究

◇概要（The abstract)◇
埼玉県越谷市をモデル地域に設定し，小児医療市場の構造分析と医療機関の戦略策定を行った。同市は，近年都心への通勤圏として整備されてきており，それに伴って０～14歳人口が全国と比較して多いことから小児医療提供体制の整備が必要である。医療機関の機能分化の推進により，同市の診療所と病院の医療サービスに差異が生じており，患者のニーズに対応したさらなる病診連携が必要になってくるものと考えられた。また，プライマリ・ケア，２次救急，高次機能の細分化による一歩進んだ機能分化のシステム構築が必要と考えられ，医療機関に応じた改善点や戦略策定が重要であると考えられた。特に，診療時間，院外処方箋発行，診療科目，IT化，および患者サービスを戦略策定のキーワードとして考える必要があるものと思われた。
Koshigaya City has a growing population and a higher birth rate than the national average. We propose a strategy for pediatric medical institutions based on structural analysis in Koshigaya City. One conclusions are as follows:
1. The strengths of the hospital in Koshigaya City were found to be intensive care, team medicine, and outpatient treatment. On the other hand, it weaknesses were non-selective medical provision which was inconvenient for patients.
2. The strength of the clinic was its ready accessibility for patients because of a wide choice of consultation hours and days. The weaknesses were its shortage of pediatric experts and low rate of out-hospital prescriptions.
3. The need for preventive medicine, wellness, disclosure, and information technology is pressing in Japan. These demands were being addressed by both the hospital and the clinic in Koshigaya City.
4. We propose a pediatric medical service strategy for both the hospital and the clinic on the basis of the distinction and the regional positioning.
